The Collaborative had a busy and exciting start to the fall season! Following our all-member meeting in September, we came together to submit comments to the New Mexico Health Care Authority Medical Services Division regarding its request for information relating to the development of a Rural Health Transformation Plan in support of the grant application for the Rural Health Transformation Program (RHTP). 40 Collaborative members worked together to develop this RFI with several additional members providing written feedback. The Collaborative also worked together to advise on the YesNMConnect Logic Model. Feedback on the logic model was received by HCA and the UNM PRC team in September. A version of the logic model with the integrated feedback will be shared at the next Closed-loop Referral Systems/Data, Technology and Standards meeting.

The kickoff session for the Rooted in Resilience: Healing the Circles within Indigenous Social Drivers of Health Co-learning Series took place on October 23rd with an in-person session. Members came together to define social drivers of health and share examples within New Mexico. View the slides here. A report from this session will be available January 2026. The next session will take place on November 20th at 10 am MT.
