Rooted in Resilience: New Mexico Indigenous Social Drivers of Health Yucca Framework
This community-developed framework outlines the importance of Indigenous Social Drivers of Health (ISDOH) in New Mexico, highlights community-developed definitions of health, and advocates for a shift in public health systems. The Yucca offers a visual of the things that help us live a balanced and well life.
This framework is a tool meant to be adapted for various audiences such as teachers, healthcare workers, Tribal Leaders, policymakers, community members, and others.
Here are some ways the co-creators of this framework are planning to apply its teachings:
- Create an onboarding training for clinicians about the role of Indigenous Social Drivers of Health in patient care.
- Share the framework with our local health councils to use in advocacy work
- Provide this as a tool for community health workers to talk about all the things that contribute to health and well-being.
- Connect with the organizations listed in the community highlights each social driver of health.
We invite you to engage with the framework and apply what is offered in your own workplaces and community spaces. If you have any feedback, please submit it here. This document will continue to grow and highlight the incredible work of our Pueblos, Tribes and Nations in New Mexico.

December 2025 All-member Session Slides
At our all-member session in December, we heard from four presenters about their closed-loop referral systems and recommendations for how the Collaborative can support these systems in the future.

Description: At this virtual session we heard from Dr. Yolandra Gomez, Bryce Townsend, Kirsten Hansen and Jasmine Meyer about their approaches to integrating Indigenous Social Drivers of Health within the healthcare system.
Goal: Identify and share examples of strategies to Integrate Indigenous Social Drivers of Health within the Healthcare system.
Objectives
- Recognize that an ISDOH framework can address unique cultural and contextual needs and present their effect on health and wellness for American Indian and Alaska Native communities.
- Describe ways these new understandings can inform public health practice.

Open & DownloadAddressing the Social Determinants of Health through a Closed-Loop Referral System Environmental Scan
The purpose of this report is to inform the development of a closed loop referral system(CLRS) for the State of New Mexico. The report highlights findings from examining current CLRS efforts across New Mexico, engaging the New Mexico Social Determinants of Health Collaborative(NMSDOH-C) in interviews and a landscape assessment survey, and identifying recommendations from states currently implementing CLRS. This report is a first step in an ongoing effort to bring together community voice to inform the healthcare delivery system and decision-makers in New Mexico and address barriers in connecting individuals to preventive services, social services and treatment.
